Please fill out the following to request any changes to your policy. You may request adding an authorized person, an address change, and adding a DBA. Requests will be processed as quickly as possible and we will email you these updated documents as soon as they are completed. Requests will be processed M-F 7AM -8PM EST, if you send in a request over the weekend or holiday it will be processed on the following workday.
